If you have knee osteoarthritis, you have probably heard mixed things about laser therapy. Some people swear by it, some studies show nothing. The frustrating part is that both can be true at once, and the reason why is the single most useful thing to understand before you try it.
What laser therapy is
Laser therapy, also called photobiomodulation (PBM) or low level laser therapy (LLLT), uses red and near-infrared light to support the body's own repair and recovery processes. It is non-invasive, contact-free, and painless, and it is not a surgical or cutting laser.
What the evidence shows, and the catch
Here is the honest picture. Earlier reviews of laser therapy for knee osteoarthritis came to conflicting conclusions, and that inconsistency is exactly why the therapy has a mixed reputation.
Then a systematic review and meta-analysis published in BMJ Open in 2019 looked more carefully at the dose. It found that laser therapy reduced both pain and disability in knee osteoarthritis, but the benefit depended on using an adequate dose. Specifically, the positive results clustered around wavelengths of 785 to 860 nm delivered at 4 to 8 joules per treatment spot, or 904 nm delivered at 1 to 3 joules per spot, in line with international dosing recommendations.
That is a stronger result than many people expect, and it comes with a clear condition attached: the dose has to be right.
Why studies disagreed, explained simply
Think of it like antibiotics. The right drug at the wrong dose will not do the job, and you might wrongly conclude the drug does not work. Laser therapy is similar. When trials under-dosed, or applied the light to too few points, they often found little effect. When they followed recommended dosing, results improved.
