What Is Photobiomodulation and Why Does It Keep Appearing in Medical Research?

Photobiomodulation keeps appearing in clinical journals, oncology guidelines and fertility research. Here is what it actually is and why the medical world is paying attention.

You have probably seen it mentioned in a fertility forum, a chronic pain support group or a sports recovery article. Photobiomodulation. PBM. Red light laser therapy. It is one of the most extensively researched non-invasive therapies in modern medicine, and most people have never heard of it.

The name that took sixty years to get right

Photobiomodulation has had five official names since its discovery in 1967. It started as laser biostimulation, the term Hungarian surgeon Endre Mester used when he accidentally discovered that low-powered laser light stimulated wound healing and hair regrowth in mice. As the therapy moved westward it became cold laser therapy, then low level laser therapy (LLLT). In 2014, the major international photomedicine organisations agreed on a single standardised term, photobiomodulation.

Why it keeps appearing in medical journals

A 2025 umbrella review covered more than 9,000 patients across 15 health conditions. It found consistent positive outcomes for PBM in knee osteoarthritis, fibromyalgia, neuropathic pain, post-surgical recovery and several other conditions. PBM is now formally included in international clinical oncology guidelines for oral mucositis, the painful mouth inflammation caused by chemotherapy.

What it actually does inside the body

Inside every cell is the mitochondria. At the end of the mitochondria's energy production chain sits a protein called cytochrome c oxidase. This protein absorbs specific wavelengths of red and near-infrared light. When photons at the right wavelengths reach it, they activate it. That activation triggers increased energy production, improved circulation, activation of the cell's own repair and anti-inflammatory programs, and changes in gene expression that keep the repair process running for hours to days after the light session ends.

Is there solid evidence or is it still experimental?+

Both, depending on the condition. For knee osteoarthritis, fibromyalgia, tendinopathy and oral mucositis the evidence is robust. For fertility and neurological conditions it is growing strongly.

Why does wavelength matter in PBM?+

The protein PBM targets inside the mitochondria has specific absorption peaks at particular wavelengths. Light outside those peaks does not trigger the cellular response.
